This page is used to inform visitors regarding our policies with the collection, use, and disclosure of Personal Information if anyone decided to use our Service


What data do we collect from you?


We will not collect, share or control your personal information for purposes other than those described in this privacy policy. You can decline to submit your personal information, in which case the Company may not be able to provide you with part or all of the Services.  The information that we collect and process about you includes:


The information you give us-


  • Personal and contact details such as Name, Date of birth, Telephone number, email address, your hospital number and your account log in credentials.
  • Any data entered by you on the questionnaires periodically sent to you by your treating clinician/ hospital/ GP surgery about your health status and disease specific symptoms as required and determined by the clinician treating you.
  • When you provide your information to us directly via our website/app, over the phone or face to face we only ask you to supply information that we need in order to provide the service you have requested. We will normally ask you to provide us with:
  • Your name
  • The organisation you work for
  • A work related email address
  • A work related telephone number
  • Account log in credentials
  • Trouble shooting and support data

However, if you do business with us, we may also ask for further information about you or your company including finance details.


The information we will collect about you-

  • Any data entered by your treating clinician/ Healthcare provider such as your Hospital/ NHS number, date and details of any treatment provided by your clinician and any other relevant clinical information as determined by your clinician/ healthcare provider that they wish to enter and store about you.



  • Log Data


We want to inform you that whenever you use our Service, in a case of an error in the app we collect data and information (through third party products) on your phone called Log Data. This Log Data may include information such as your device Internet Protocol (“IP”) address, device name, operating system version, the configuration of the app when utilizing our Service, the time and date of your use of the Service, and other statistics.


  • Cookies


Cookies are files with a small amount of data that are commonly used as anonymous unique identifiers. These are sent to your browser from the websites that you visit and are stored on your device’s internal memory. This Service does not use these “cookies” explicitly. However, the app may use third party code and libraries that use “cookies” to collect information and improve their services. You have the option to either accept or refuse these cookies and know when a cookie is being sent to your device. If you choose to refuse our cookies, you may not be able to use some portions of this Service.


Why do we collect this data?


We only collect the minimal data set required for the adequate function/ maintenance/ service of the product and provide the service needed for you as the end user and your healthcare provider in managing your clinical condition. We need to collect your data to validate you are the authorised user for the product, collect information as determined and required by your healthcare provider in managing and treating your condition. We will not use your data for any marketing purposes without your explicit consent. No data is collected outside what is stipulated in this privacy policy. The privacy policy will be updated if there is a change in the purpose of data collection and users will be asked to re-sign the privacy policy.


How do we process your data?


Any data about your health condition you enter on the questionnaires will be analysed based on the clinical thresholds set by your healthcare provider to make it easier for them to process and analyse your data. Anonymised data may be used for research purposes to further improve our systems. Access to your personal information is strictly controlled on a ‘need to know’ basis. The amount of access to your data required among users in the healthcare provider and customer support staff is predetermined according to their role and access restricted accordingly.


Data security and data storage


We value your trust in providing us your Personal Information, thus we are striving to use commercially acceptable means of protecting it. But remember that no method of transmission over the internet, or method of electronic storage is 100% secure and reliable, and we cannot guarantee its absolute security. Any data you enter on the app/webpage is encrypted in transit and at rest. Your data is stored in secure Amazon servers located in the UK and backed up regularly.


What is the legal basis of collecting your data?


We rely on the following legal bases to use your personal information:


A. Where it is needed to provide you with our products or services, such as:


  • Managing products and services you hold with us, or an enquiry about one


  • Updating your records, tracing your whereabouts to contact you about your account and doing this for recovering debt (where appropriate)


  • Sharing your personal information with third parties and services providers when you apply for a product or service to manage your product or service


B. Where it is in our legitimate interests to do so, such as:


  • Managing your products and services, updating your records


  • To perform and/or test the performance of, our products, services and internal processes


  • To refine / improve our product and service to our customers and patients


  • To follow guidance and recommended best practice of government and regulatory bodies


  • For management and audit of our business operations including accounting


  • To carry out monitoring and to keep records of our communications with you and us


  • To administer our good governance requirements and those of other members of our Group, such as internal reporting and compliance obligations or administration required for AGM processes


  • Where we need to share your personal information with people or organisations in order to run our business or comply with any legal and/or regulatory obligations


  • To comply with our legal obligations or where we reasonably believe we are under a legal or regulatory obligation


C. Implied consent

  • Consent is implied where information is collected for the purposes of direct care*. This includes all the information you are giving us by answering the questionnaires and personal information given to us upon request by your healthcare providers.


D. With your consent or explicit consent

  • For some direct marketing communications and digital advertising activities
  • Transfer of the whole of your identifiable records to a third party


What rights/controls do you have over the data we collect?


Remcare has been designed and produced in keeping with the GDPR regulations. You have a right to withdraw your consent at any time.


Your right to access the information we hold about you:


With some exceptions designed to protect the rights of others, you have the right to a copy of the personal information that we hold about you, as well as information about what we do with it, who we share it with and how long we will hold it for. We may make a reasonable charge for additional copies of that data beyond the first copy, based on our administrative costs. This information has to be requested in writing and enough information must be given to us to identify you with your personal identifiable information


  • Your right to have inaccurate information about you rectified.


  • Your right to object to what we do with your data, and to have restrictions placed upon it


  • Where we process your personal information on the basis of our legitimate interest, you have the right to object to that processing and to have restrictions placed upon it while we consider your objection.


  • Your right to restrict the sharing of information with third parties.


  • You have the right to require us to delete the information that we hold about you if it is no longer necessary for the purpose we collected it for, and there is no other legal basis on which we must, or are allowed to, retain it.


  • We may decline to process requests that are unreasonably repetitive or systematic, require disproportionate technical effort, jeopardize the privacy of others, or would be extremely impractical, or for which access is not otherwise required.


  • You need to contact your treating healthcare provider (who has requested you to use this product) to service your requests for deleting or making changes to your data.


  • You have a right to request that you are not subject to a decision based solely on automated processing including profiling.


Data standards and management


Your data will be stored on our data centres for a period no more than 5 years. Your data will not be accessible publicly. It is backed up regularly on secure servers in the UK. When it is deemed that we no longer need to hold your data, all the data will be deleted from our servers and back up facility.

If you choose to use our Service, then you agree to the collection and use of information in relation to this policy. The Personal Information that we collect is used for providing and improving the Service. We will not use or share your information with anyone except as described in this Privacy Policy.

Information Collection and Use

For a better experience, while using our Service, we may require you to provide us with certain personally identifiable information, including but not limited to Analytics on use of the app content, location of use and number of downloads. The information that we request will be retained by us and used as described in this privacy policy.

The system does not use third party services outside the Amazon Web Services infrastructure.

PDS FHIR API


If you are receiving care from a health or care organisation, that organisation may share your NHS number with other organisations providing your care. This is so that the health and care organisations are using the same number to identify you whilst providing your care. By using the same number the health and care organisations can work together more closely to improve your care and support.


Your NHS number is accessed through an NHS England service called the Personal Demographic Service (PDS). A health or care organisation sends basic information such as your name, address and date of birth to the PDS in order to find your NHS number. Once retrieved from the PDS, the NHS number is stored in our case management system. These data are retained in line with our record retention policies and in accordance with the Data Protection Act 1998, Government record retention regulations and best practice. We will share information only to provide health and care professionals directly involved in your care access to the most up-to-date information about you. Access to information is strictly controlled, based on the role of the professional, and where the user has a direct care relationship with you.


The use of joined up information across health and social care brings many benefits. One specific example where this will be the case is the discharge of patients into social care. Delays in discharge (commonly known as bed blocking) can occur because details of social care involvement are not readily available to the staff on the hospital ward. The hospital does not know who to contact to discuss the ongoing care of a patient. The linking of social care and health information via the NHS number will help hospital staff quickly identify if social care support is already in place and who the most appropriate contact is. Ongoing care can be planned earlier in the process, because hospital staff will know who to talk to.


You have the right to object to the processing of your NHS number in this way. This will not stop you from receiving care, but will result in the benefits outlined above not being realised. To help you decide, we will discuss with you how this may affect our ability to provide you with care, and any other options that you have.

NHS login


Please note that if you access our service using your NHS login details, the identity verification services are managed by NHS England. NHS England is the controller for any personal information you provided to NHS England to get an NHS login account and verify your identity, and uses that personal information solely for that single purpose. For this personal information, our role is a “processor” only and we must act under the instructions provided by NHS England (as the “controller”) when verifying your identity. Children’s Privacy


These Services will be used for children undergoing treatment if the child’s healthcare provider uses this service. The child’s parent/guardian will be responsible for all information entered on the system. All the details in this privacy policy will be applicable to children’s information as well. If you are a parent or guardian and you are aware that your child has provided us with personal information accidentally, please contact us so that we will be able to do necessary actions.


Changes to This Privacy Policy


We may update our Privacy Policy from time to time. Thus, you are advised to review this page periodically for any changes. We will notify you of any changes by posting the new Privacy Policy on this page. These changes are effective immediately after they are posted on this page.

You will be onboarded, with your consent, from your existing details on Remcare if a new health care provider/ hospital who are treating you wish to use Remcare for your care. This will mean that your new healthcare provider/hospital can use the details stored by Remcare to save your new healthcare provider/hospital time in sending you your questionnaires.

If you wish to make any changes to your data on the app in your line with your GDPR rights, please contact your healthcare professional who has requested you to use the app. They have full access to your data and have the ability to make any changes as requested by you. Your rights


The data that we keep about you is your data and we ensure that we keep it confidential and that it is used appropriately. You have the following rights when it comes to your data:


  • You have the right to request a copy of all of the data we keep about you. Generally, we will not charge for this service;


  • You have the right to ask us to correct any data we have which you believe to be inaccurate or incomplete. You can also request that we restrict all processing of your data while we consider your rectification request;


  • You have the right to ask that we erase any of your personal data which is no longer necessary for the purpose we originally collected it for. We retain our data in line with the Information Governance Alliance’s guidelines (Records Management Code of Practice )


  • You may also request that we restrict processing if we no longer require your personal data for the purpose we originally collected it for, but you do not wish for it to be erased.


  • You can ask for your data to be erased if we have asked for your consent to process your data. You can withdraw consent at any time – please contact us to do so.


  • If we are processing your data as part of our legitimate interests as an organisation or in order to complete a task in the public interest, you have the right to object to that processing. We will restrict all processing of this data while we look into your objection.


You may need to provide adequate information for our staff to be able to identify you, for example, a passport or driver’s licence. This is to make sure that data is not shared with the wrong person inappropriately. We will always respond to your request as soon as possible and at the latest within one month.
