What is Remcare?


  • Remcare is a secure web application (web-app) which allows you to complete your pre-operative health questionnaire online. 
  • Remcare is an NHS-approved secured web-app that provides this service to patients on behalf of your local hospital.

How do I access Remcare?

If you received a text message with the link, follow the step-by-step guide to register:


  1. You should receive a text message from the healthcare provider that contains a URL to Remcare’s onboarding pages that are unique to you and your account.
  2. Click on the link, and you will be taken through a series of onboarding consent pages before we display your or your child’s details. 
  3. Add the DOB of the person's name that appears on the screen in the format. DD/MM/YYYY.   
  4. Check your text message for the unique code, and enter the codes provided in the text message into the ‘Enter code’.
  5. Click activate account 
  6. You will be directed to the “NHS login” or “Create Remcare password”.  * If the patient is less than 11 years old the “NHS login” menu will NOT appear on the screen. This is because the NHS login does not support any patients under 11 years of age.  
  7. If you click the “create Remcare password”, add your and confirm new password and click Set password.


    Your account is now active! 


*Your username is the mobile number that you received the text invitation from your healthcare provider.

*If you clicked “continue to NHS login”, see the section below on registering with NHS Login.

 

The link in your text invitation will not expire unless you clicked the link and aborted the onboarding process.  You will receive an automated reminder on Day 1, 3 and 7 if you have an incomplete questionnaire.   If you lose or delete the text message, the healthcare provider who invited you can send the text invitation which will resend you the link.

Registering with NHS login 


This step-by-step guide is for patients who wish to register or continue with the “NHS Login” 


  1. You will be directed to the NHS login sign-in page 
  2. Enter your email and click continue  *If your email is not linked to an existing NHS login, you will be automatically taken through the steps to create one.  See the section below on how to create an NHS Login.
  3. Enter your NHS login password & you will be sent a One Time Password (OTP) to your mobile phone. 
  4. Enter the OTP and click continue 
  5. You can now access your Remcare account using your existing NHS Login details! 

I aborted the onboarding pages after I accepted the consent pages and I do not have a password? 


As soon as the link is used, it will become invalid, meaning no one else can use it to create a record – it is unique to you. 

If you make a mistake, abort the onboarding process or your link is expired, you can continue by using the URL https://remcare.org/sign-in 


*Your username is the mobile number that you received the text message from your healthcare provider. 

*Your username is the email address that you received the email invitation from your healthcare provider. 


  1. Click reset password 
  2. You will receive a text message or an email to reset your password 
  3. Create a new password 
  4. You can access Remcare using your username and new password
  5. You will be taken through a series of onboarding consent pages before we display your or your child’s details. 
  6. Add the DOB of the person's name that appears on the screen in the format. DD/MM/YYYY.   
  7. Check your text message or an email for the unique code, enter the codes provided in the text message or an email into the ‘Enter code’ 
  8. Click Activate account 
  9. You will be directed to the “NHS login” or “Create Remcare password”.                                                             * If the patient is less than 11 years old the “NHS login” menu will NOT appear on the screen. This is because the NHS login does not support any patients under 11 years of age. 
  10. If you click the “create Remcare password”, add and confirm the new password and click Set password. 


Your account is now active! 


11. If you click “continue with the NHS Login” 

  1. You will be directed to the NHS login sign-in page 
  2. Enter your email and click continue.                                                                                                                *If your email is not linked to an existing NHS login, you will be automatically taken through the steps to create one.  See the section below on how to create an NHS Login.
  3. Enter your NHS login password & you will be sent a One Time Password (OTP) to your mobile phone. 
  4. Enter the OTP and click continue 

 

You can now access your Remcare account using your existing NHS Login details! 

Creating an NHS login 


If you do not have an existing NHS login, you will be prompted to create one after entering your email from the NHS login sign-in page.  NHS England Digital have published a video and step-by-step guide on how patients can do this, you can view the video below:

How to sign into Remcare directly using your NHS login?


This step-by-step guide is for patients with a registered Remcare record: 

  1. Click 'Continue to NHS login' from the  Remcare sign-in page 
  2. You will be directed to the NHS login sign-in page 
  3. Enter your email and click continue 
  4. Enter your NHS login password and you will be sent a One Time Password (OTP) to your mobile phone. 
  5. Enter the OTP and click continue 
  6. You will now be signed into Remcare. 

 

How to sign into Remcare (for patients with a registered Remcare record)?


This step-by-step guide is for patients with a registered Remcare record: 


*If your healthcare provider sent you

  • a text invitation, then your username is the mobile number that you received the text invitation
  • an email invitation then your username is the email address that you received the email invitation


  1. Add your username and password and click sign in 
  2. Check your text or email for the unique code, enter the codes provided in the text message or email invitation into the ‘Enter code’ 
  3. Click send. 


 You can now access your Remcare account.

If the child is a Local Authority Child (LAC) and the social worker would complete the questionnaire on behalf of the foster parent:


If the child is a Local Authority Child on the waiting list and their foster carer had initially activated the link, however, they do not have parental authority to complete the questionnaire/documents but can only accompany the child to the appointment.  Then the child's social worker can complete the online Remcare questionnaire on their behalf.   


The healthcare provider's administrator must update the child’s email address to the social worker’s email address but leave the foster parent’s mobile number so that the foster parent can continue to receive the Remcare message from the healthcare provider’s clinicians relating to their appointment.


  • If the child’s account is inactive, the social worker will get the link to activate the account to complete the online Remcare questionnaire.


  • If the child’s account has already been activated by the foster parent, the social worker will not get the link, so after the healthcare provider's administrator has updated the email address to the social worker’s email address, the social worker MUST click on the link  https://remcare.org/sign-in to reset their password.  The section “I aborted the onboarding pages after I accepted the consent pages and I do not have a password?” above on this page gives a step-by-step guide on how to access Remcare if an account is already activated.

Why have I been asked to fill in a Remcare Health Questionnaire?

 

You or your child are on the waiting list for a procedure at the hospital.  The hospital has introduced a digital pathway for patients.  You must complete the Remcare health questionnaire as soon as possible to assess if you are or if your child is okay to proceed with surgery. Please do NOT wait for a surgery date, the sooner you complete the questionnaire, the sooner we can plan what happens next.


Do I have to complete the Remcare Health Questionnaire to have an operation?


Remcare web-app is designed to be user-friendly so we would ask you to try to complete it when possible. You may wish to ask a trusted family member or friend to assist you if you feel able to share medical details. If you do not have anyone to help please don’t worry, it does not mean your operation will be cancelled.


If you cannot complete your Remcare Health Questionnaire, please contact the healthcare provider (their contact detail is in the text or email) who would arrange for a nurse to go through the questionnaire in person or over the telephone.

Do I have to complete the Remcare Health Questionnaire if I have been given a To Come In (TCI) date before my appointment?


Please complete your Remcare health status questionnaire as soon as possible because completing the online questionnaire:


  • is essential preparation and can help your clinicians detect any problems early on. Failure to complete the online questionnaire could therefore delay your surgery. 


  • reduces waiting times for patients and makes it easier for the hospital to plan your care.




How do I access the Remcare Health Questionnaire?

 

After you are logged on to the Remcare web-app, you will find the questionnaire tab.  In this module, you can access all the questionnaires that your healthcare provider has sent you.  The pending status are the questionnaires that you need to start completing and submit.  You can also view the completed questionnaire online or download it as a pdf file and share with your other healthcare provider if needed.



 How do I access the patient information (leaflets) on my Remcare Account?

 

After you are logged on to the Remcare web-app, you will find the patient information tab. In this module, you can access all the leaflets and videos that your healthcare provider has sent you. The videos and leaflets are customised to your treatment plan. You MUST read and view the video before your appointment so if you have any questions, the pre-assessment will be able to answer them.


How do I view and reply to the messages that my nurse or clinician has sent me on my Remcare Account?

 

After you are logged on to the Remcare web app, you will find the message tab. 

In this module, you will receive messages from your nurse and clinicians requesting further information. 

You can send messages, videos, pdf files and images to your nurse or clinician.


Does filling out Remcare Health Questionnaire mean I will receive a date for surgery soon?


Unfortunately, some surgery waiting lists are long at the moment. Filling out the Remcare Health questionnaire does not guarantee a date for surgery. You might still have a long wait before your surgery. However, completing your Remcare Health questionnaire is essential preparation and can help us detect any problems early on and ensure that you are waiting well.


What happens after completing the Remcare Health Questionnaire?

 

  • Your questionnaire will be reviewed by a nurse. This will help determine how much pre-operative assessment you will need.


  • Some people will need minimal pre-operative assessment. This is usually people who are fit and healthy having minor operations.
  • Some people will need longer pre-operative assessments. This could be a telephone call or a face-to-face visit.
  • If you have multiple medical problems, you might need to see an anaesthetist and have some extra tests before surgery.


Inform your clinician if your health changes:



If your health changes, please inform your clinician as soon as possible so they can make changes to your treatment plan. You can edit the response on your completed questionnaire, after you have edited and submitted the new response, please send a message to your clinician informing them that your health has changed since you last completed the questionnaire using the message tab on the Remcare web app.


I have already completed a Remcare Health Questionnaire. Should I complete another one?


If you have already completed a Remcare Health Questionnaire (for example for another operation) then you will need to update it, especially if your medical details have changed.


To do this, please log in using the email address or mobile number and password you registered with originally. You can update your details. Please be sure to submit the new response and send a message to your clinician informing them that your health has changed since you last completed the questionnaire using the message tab on the Remcare web app.


There may be times when your nurse or clinician may ask you to complete a fresh questionnaire if you have completed a questionnaire a long time ago.


Who can I contact for help with Remcare?


If you are struggling with Remcare, you can contact your pre-operative assessment clinic and they will be able to hopefully guide you through the process.


If you have a clinical query relating to your operation (i.e. you no longer need the operation) you should contact your surgeon's secretary.

6.   Two-way messages - Patients and the clinical teams can send secure messages (Text, pdf file, image and video) through the Remcare Web app when needed.
